Registration Information and Forms

Camp Quest Chesapeake will be held on the shores of the Patuxent River in Maryland, about 20 miles east of Washington, DC. Camp will run from July 14-20, 2013. Kids must be at least 8 years old on the first day of camp to attend. Campers ages 16 and 17 will be counselors-in-training (CITs).

Registration for Camp Quest Chesapeake 2013 is now closed, as we have already reached capacity. You might consider registering your camper at Camp Quest Ohio, which is the nearest Camp Quest to CQC.

Already registered? Please be sure you've completed all of the steps below. Also, please see our Parent Information Page for additional information about camp.

Steps for Registered Campers

1. Submit the health forms required for registered campers by June 14.
   • Download and mail in the Health Forms. (Please note that the physician's form must be filled out by your child's physician.)
2. Submit a color photo of your child by June 14.

• Send the photo by mail to:

3. Pay any remaining balance by June 14, using one of these methods:
